Summary

Under the direction of a NYS licensed Physical Therapist, provides physician ordered physical therapy services. Coordinates the physical therapy plan with the interdisciplinary team care plan.

Essential Responsibilities/Accountabilities

Provides physician ordered physical therapy services as part of a care plan developed by a NYS licensed Physical Therapist.
Is responsible for completing, in a timely manner, all documentation including visits notes, renewal orders, and verbal orders.
Reflects progress or lack of goal achievement in all charting. Confers with physician and/or interdisciplinary team regarding patient response to therapy.
Evaluates home environment for changes needed to increase patient's independence. Recommends equipment and assistive devices (ramps, bathroom aides, adaptive wheelchair) as appropriate.
Instructs patients and caregivers in therapeutic exercises, which may be used to improve the patient's functional status.
Provides gait and ambulation training. Selects and instructs in the use of various assistive devices (orthotic appliances, crutches, walker, cane, etc.).
Treats patient's physical deficits using clinically accepted therapeutic exercises. Specifies technique used in the plan of care. Communicated regularly with Physical Therapist (who oversees each patient's Physical Therapy Plan of Care) regarding patient status.
Instructs patient and caregivers in safe transfers (bed, chair, commode etc.) using appropriate body mechanics and equipment (sliding board, Hoyer lift, trapeze, bath bench, etc.).
Attends scheduled interdisciplinary staff meetings and actively participate in treatment planning.
Participates in committees and program development, as needed.
Consistently demonstrates high standards of integrity by supporting the Companies' mission and values and adhering to the Corporate Code of Conduct.
Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
Regular and reliable attendance is expected and required.
Performs other functions as assigned by management.

Minimum Qualifications

Associates degree from an accredited Physical Therapist Assistant program and current NYS PT Assistant licensure required.
Minimum of 2 years post-graduate experience as a PT Assistant in an acute care facility and/or an acute intermediate rehabilitation facility.
Must have ability to travel.
Must demonstrate strong communication, documentation, computer, multi-tasking and interpersonal skills.
Pre-employment physical examination and an annual health assessment as required by New York State Health Department.

Physical Requirements

Ability to travel as necessary in order to offer support for on-site visits to groups/brokers, training, and/or off-site meetings.
