To provide and coordinate a full array of assessment, referral, outreach and engagement, tracking, application processing, interviewing and customer services for clients in the Access to Recovery II Program. This position is a field position but will have a permanent space in an office. This position requires driving multiple miles per day. Individual must be able to obtain clearance for access to department of correction and court support services and judicial facilities/agencies.
Provide assessment, referral, and outreach and engagement services, including services that assist clients in obtaining basic supports needed to improve their likelihood of success in recovery. Including, but not limited to the following;
Assessment, referral, outreach and engagement attempts with clients in various community settings, including CSSD, DOC and other ATR II Portals agencies.
Service planning for non-clinical needs & Recovery Supports (e.g., housing referrals)
Assistance and advocacy in accessing extant community support services
Assistance in accessing urgent and standard Basic Needs supports
Face to face support
Referrals and contacts with culturally relevant organizations and individuals such as the following, as needed, to establish and maintain adequate community supports:
Vocational providers, employers,
Housing agencies, landlords
Health care providers
Probation/Parole officers
Crisis intervention services, hospital emergency departments
Other appropriate support and treatment services
Family members and other natural supports
Interpreter services
Faith Communities (e.g. churches, masjids, etc.)

Skills:
Qualifications/Education;
Bachelor’s degree in Human Services or related field, with experience in behavioral health disorders
Or
Associate degree in Human Services or related field
And
Two years experience in substance abuse and co-occurring disorders
Individual must be able to obtain clearance for access to department of correction and court support services and judicial facilities/agencies.

Knowledge of substance abuse and mental health services
Knowledge of community resources in the local region
Knowledge of Cultural Diversity, Engagement, and Ethics
Computer skills
Strong prioritization and organizational skills Bilingual preferred
