The Clinical Case Manager will visit our senior members in their homes to coordinate their care. When not visiting patients, the chosen candidate can make calls and do the necessary documentation from the comfort of his or her home office. Key responsibilities of the Clinical Case Manager include:
- Managing the care coordination for our senior members
- Visiting members in their homes
- Documenting cases via laptop
- Providing telephonic care management
- Following up with patients
- Identifying costs effective packages of services for members while still meeting their healthcare needs
- Educating members and their caregivers on their health

Experience: Minimum requirements include: - A current, unrestricted RN license in your state of residence
- A minimum of 2 years of clinical experience as an RN,
- Computer proficiency
- Excellent communication and organization skills
RNs with one or more of the following will be preferred: - Public Health Nurse (PHN) credentials or a BSN degree
- Medicare and/or Medicaid experience
- Clinical experience in med/surg; geriatric care; nursing home; assisted living; home care; or public health
- Experience working with the low-income population
- Bilingual (Spanish, Somali, Russian or Hmong) skills
- Familiarity with Microsoft Word
