Establish and maintain positive relationships with referral sources. Conduct physical assessments of potential patients. Respresents the company w/professionalism to referral sources (i.e., case managers, discharge planners, social workers, and physicians). Completes required forms & documents in accordance w/company policy and state and/or federal regulations. Maintains a strong working relationship w/attending physicians in area hospitals to promote services.

Education/Qualifications:

RN Preferred - must have one year experience in patient/resident assessments; Must be able to travel 75% of the time on day trips to multiple hospitals in the are; Must have excellent communication and presentation skills; Must have skills to conduct basic sales/marketing presentations; Must understand the admissions process from acute care to long-term, skilled nursing care.
