Qualified candidates should possess: - Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) -  Certification as a certified Advanced Social Worker Case Manager (CASWCM) by the National Association of Social Workers, or possess a minimum of 3 years experience within the preceding 5 years as a Social Worker Case Manager in a hospital-based setting performing medical crisis counseling, discharge planning and/or working in Pediatrics, Adolescent or Internal Medicine. - Valid, unrestricted license or certification to practice Clinical Social work in any one of the fifty states, the District of Columbia, Puerto Rico, Guam, or the US Virgin Islands.
