Social Worker

Reports To: Director of Knickerbocker Village NORC

Responsibilities:

The KVSS NORC Social Worker is responsible for providing case management and case assistance and the development and maintenance of care plans in both Chinese and English languages, including the following services:
- Information and Referral
- Financial planning
- Advocacy
- Benefit and entitlement assistance

The KVSS NORC Social Worker is also responsible for providing individual counseling and group work in Chinese and English for the senior residents of Knickerbocker Village. This includes but is not limited to addressing depression, anxiety, bereavement, family reconciliation, dementia, maladaptive behaviors and cross-cultural barriers and stressors.

The KVSS NORC Social Worker will assist clients in the coordination of care and services from various home health care agencies, benefit and entitlement agencies and legal and financial providers.

The KVSS NORC Social Worker will participate in weekly staff meeting and case conferences. S/He will participate in NYC DFTA and agency trainings as necessary and appropriate. S/He will conduct home visits and assessments. S/He will assist in the planning and implementation of new educational and recreational activities for the members and residents of the Knickerbocker Village NORC. As well as other duties as assigned.

Skills:
Qualifications:

- Masters degree in one of the following fields: social work, counseling, psychology, gerontology.
- Enjoy working with seniors
- Two years experience of direct practice with senior population
