Provide nutritional/dietary consultations to participants in the patient management programs. Primary duties may include, but are not limited to: Ensures patient understanding of nutritional goals and objectives and provides nutritional counseling and patient education to positively impact outcomes for individuals diagnosed with chronic conditions. Reviews the dietary needs of identified patients by using 3-day food diary, assessments, and other available tools (including nurses' assessment of patient's issues). Determines best ways to motivate participants to make changes/enhancements to their current nutritional plan. Identifies areas requiring improvement and consult with patients via telephone to initiate behavior change related to dietary intake and management. Interfaces and communicates with physicians or other health professionals as needed to ensure adherence to prescribed plan of care. Assists with staff development and training to equip nurse care managers with the expertise and knowledge needed to manage the patient's total healthcare needs. Develops training modules as needed for use by the nurse consultants in providing care management services for participants. Provides train-the-trainer program to ensure understanding of the training modules and nurses' understanding of the tools. Delivers and participates in training programs for newly hired employees as needed.
Skills:
Requires registered dietitian with a minimum of 3 years experience counseling individuals regarding nutritional/dietary management issues related to chronic disease (primarily diabetes, heart disease, and pregnancy) or any combination of education and experience, which would provide an equivalent background. Previous patient management/ customer service experience required.
