Holy Rosary in Miles City, MT is seeking a temp Social Worker, M-F days split between Acute Care and Hospice. Social Worker will be FT mainly M-F days, depending upon the needs of the departments. It will be split between Acute care and Hospice. Travel to and from hospice visits to be reimbursed by facility at federal rate at the end of the month. A reliable vehicle is required as hospital vehcile is not always avilable. Assists patients and their families to resolve and/or deal with the emotional, social and financial impact of diagnosis, illness and change in lifestyle. Evaluates psychosocial functioning of the patient and family in regards to the patient support system. Supports the patient and family development of a life plan; counsels and provides behavioral health therapy as necessary. Evaluates psychosocial functioning of patients/families and provides a wide range of psychosocial evaluation, assessment, counseling therapy crisis intervention. Helps patients with advance medical directives. Identifies and involves community services as needed. Provides consultation and education to staff and volunteers.
REQS: 1.MSW Degree in Social Work from an accredited college or university. 2. Licensed Clinical Social Worker (LCSW) Licensure as required by the state of Montana, preferred or working toward licensure. 3.Must possess a valid Montana Driver s License and proof of liability insurance. 4.Requires two years experience in medical social work, preferably in hospice or home health setting. 5.Special skills necessary are advanced clinical social work, communication, counseling, and casework.
