Administers skilled physical therapy treatments, individual and group, according to the established Plan of Care, utilizing appropriate procedures, equipment and techniques within the scope of practice of physical therapy.Conducts a thorough initial assessment of the patient to determine functional abilities/deficits and rehabilitation potential. Develops and implements an individualized Plan of Care containing achievable/functional goals, timeframes and treatment methods. Provides and documents initial and ongoing patient/family education as appropriate.
