Progressive 113 Bed Acute Care Community Hospital in Upstate New York is currently recruiting for a full-time Decentralized Pharmacist (Job Title = Patient Care Pharmacist). This pharmacist will be required to collaborate effectively with all health care providers in the patient care setting. The Pharmacy team is supported by an integrated Information system which links the pharmacy to lab and patients medical record and by the PYXIS profile, which minimizes distribution and allows the pharmacist more time to utilize their Clinical skills and judgment. This impressive facility is an ANCC Magnet designated Hospital for nursing excellence and is southeast of Rochester, off I-90. Specific Roles and responsibilities include, but are not limited to: Renal Dosing by protocol IV to PO Conversion Order Clarifications by protocol Student rotations We currently are directly affiliated with St. John Fisher School of Pharmacy, Albany and University of Buffalo. We are most active with St. John Fisher and have had 5 students (PPE-II & Advanced) so far this year and are expecting 4 more before years end. Pharmacists are responsible for documenting all clinical interventions in an intervention database. These interventions include such activities as Pediatric dosing, Antibiotic monitoring, quality initiatives, direct patient contact, and others Long Term Care.....We have a long term care facility which has a hybrid supply system (partially us and partial a vendor pharmacy). We do clinical consults on all new admissions and all significant status changes. Committee Work (P&T, Discharge planning, Nursing QI, others) Out-patient Pharmacy service is limited to same-day surgery and employee prescriptions and OTC sales We have a strong Community commitment and provide numerous opportunities to do provide community benefit and education. This opportunity will offer a days/evenings rotating schedule with alternating weekends required. Outstanding benefits offered. New York State Pharmacist licensure or eligible required.
