This role can offer the right individual the flexibility to be able to create his or her own schedule.

• Visiting various hospitals within an assigned territory to conduct onsite clinical reviews for members in the New York City, NY area.


• Evaluating delays in care, facilitating discharge planning, and arranging for alternative care services


• Managing NICU patients during their hospital stay


• Documenting all activity


• Contacting referred members and introducing the program


• Reviewing hospital census reports


• Collaborating with other clinical staff to enhance the quality and efficiency in healthcare delivery


• Educating and supporting the family unit


• Assesses and interprets customer needs and requirements.


• Identifies solutions to non-standard requests and problems.


• Solves moderately complex problems and/or conducts moderately complex analyses.


• Works with minimal guidance; seeks guidance on only the most complex tasks.


• Translates concepts into practice.


• Provides explanations and information to others on difficult issues.


• Coaches, provides feedback, and guides others.


• Acts as a resource for others with less experience.

• Candidates must possess a current and unrestricted RN license


• 2+ years of experience in a hospital setting managing high-risk / level III NICU patients


• Superior Communication skills a must


• Case management experience a plus


• Preferred Certified Case Manager (CCM)


• Solid computer skills are essential.
