Guiding referred members through their high-risk pregnancies and guiding parents during their infants' neonatal care. This role can offer the right individual the flexibility to be able to create his or her own schedule. - Visiting various hospitals within an assigned territory to conduct onsite clinical reviews for members in the New York City, NY area.
- Evaluating delays in care, facilitating discharge planning, and arranging for alternative care services
- Managing NICU patients during their hospital stay
- Documenting all activity
- Contacting referred members and introducing the program
- Reviewing hospital census reports
- Collaborating with other clinical staff to enhance the quality and efficiency in healthcare delivery
- Educating and supporting the family unit
- Assesses and interprets customer needs and requirements.
- Identifies solutions to non-standard requests and problems.
- Solves moderately complex problems and/or conducts moderately complex analyses.
- Works with minimal guidance; seeks guidance on only the most complex tasks.
- Translates concepts into practice.
- Provides explanations and information to others on difficult issues.
- Coaches, provides feedback, and guides others.
- Acts as a resource for others with less experience.

Experience: - Candidates must possess a current and unrestricted RN license
- 2+ years of experience in a hospital setting managing high-risk / level III NICU patients
- Superior Communication skills a must
- Case management experience a plus
- Preferred Certified Case Manager (CCM)
- Solid computer skills are essential.
