Supervised by: Dir. of Patient Care Services of the Home Care Agency where assigned when providing staffing for an agency. Positions Supervised: Licensed Physical Therapy Assistants. Interrelationships: Patients, family and other health care team members
JOB SUMMARY:
The Physical Therapist is a qualified professional who assesses, provides, supervises, and evaluates physical therapy services to patient in the home as prescribed by the attending physician.
The Physical Therapist also supervises the family and para-professionals and provides education to patients and/or family members on specific treatment and interventions to promote optimal health outcomes..
QUALIFICATIONS:
Education/Degree: Must be a graduate of a graduate of a school of Physical Therapy approved by the American Physical Therapy Association.
Training/Certification: Current professional license in North Carolina.
Knowledge/Skills/Abilities: Ability to work independently; make accurate, and at times, quick judgments; understands the application of the physical therapy process.
Experience: Preferred minimum of one year of experience as a Physical Therapist, in Home Health, or one year experience in a health care setting. New Grads welcome.
JOB FACTORS:
Physical Requirements: Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing steps, walking, exposure to temperature and humidity changes, and pick up and carry 75 pounds. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must be able to communicate effectively in writing and verbally. Must provide evidence of annual TB test and other state required tests or examinations.
Mental Requirements: Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to cope with mental and emotional stress, and demonstrate emotional stability.
Working Conditions: Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, excrement, and some adverse environmental conditions in a patients place of residence.
Transportation: Must have a current valid drivers license, auto registration card and reliable transportation.
ESSENTIAL FUNCTIONS
1. Applies the appropriate physical therapy process in the evaluation of patients/clients. 2. Establishes and implements the patient/client plan of treatment/care. 3. Provides appropriate health or wellness teaching and counseling to the patients/clients. 4. Documents per standards of physical therapy and agency policy and procedures. Completes all documentation forms accurately, completely and within designated time frame of assigned agency. 5. Maintains an accurate accounting of all supplies and equipment. 6. Follows the patient care policies and procedures of the assigned agency. 7. Communicates to the appropriate supervisor of patient care if unable to meet patients need or perform a procedure. 8. Demonstrates safe therapy skills and competence. 9. Provides direct patient care in accordance with physicians orders, plan of care and current assessment findings. 10. Reports all pertinent patient/client condition changes to the appropriate supervisor and the patients physician 11. Assists with coordination of care with caregivers, field staff, physicians, and supervisors through documentation, verbal communication and/or case conferences. 12. Ensures that all patients/clients consistently receive services as scheduled or notifies assigned supervisor and patient of delays or reschedules. 13. Treats patients to relieve pain and/or develop or restore function to maintain maximum performance. 14. Instructs patients/families in the use of prosthetics, orthotics and assistive devices utilized in the patients therapy program. 15. Instructs patients/families in the total Physical Therapy Program. 16. Attends staff meetings and agency in-services as requested. 17. Performs passive and active exercises and muscle re-education. 18. Administers resistive to motion exercises to increase patients strength. 19. Provides gait training to teach patients to ambulate safely. 20. Provides treatment prescribed with appropriate equipment, such as, infrared lamp, diathermy, hydrotherapy, and ultrasound. 21. Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests. 22.
