Responsibilities: Assume responsibility in coordinating care to assigned nursing patients. Assignment of patients will be made by the Location Director and responsibilities include, but are not limited to: 1. Initial and ongoing comprehensive assessments of the client's needs. 2. Initial paperwork set-up and explanation of nursing services to patient's family. 3. Monthly home supervisory visits to assess patient, family, environment and follow-up documentation. 4. Initiation and update of physician plan of treatment according to Company policy and applicable laws and regulations. 5. Communication with referring health care team, including condition, goals, and changes in care. 6. Third party payer nursing updates and re-authorization for continued care. 7. Acts as clinical resource for nursing staff. 8. Interview, hire, orient and supervision of staff nurses. 9. Participates in employment decisions affecting nursing staff, including termination as appropriate. 10. Participates in location administrative and clinical calls for after hours patient care. 11. Performs and provides direct patient care as appropriate and needed. 12. Assists in marketing by projecting a positive image of the Company at all times through professional behavior and positive interactions. .
