A Multi-Specialty Primary Care Facility providing services for 3 counties is expanding and needs to add a FAMILY PRACTITIONER (MD or DO) BE or BC. The approximate case load is 25-32 pts per day. The Payor Mix is Medicaid 47%, Self Pay 27%, Commercial 17% and Medicare at 9%. * LOCATED in the center of New York State, the city of Syracuse is in the beautiful Finger Lakes Region near Lake Ontario and Lake Oneida. A short distance from Rochester, Albany, Canada, the St. Lawrence River, the Adirondack Mountains and the Thousand Islands. There is a large selection of year round sports which includes many snow skiing and summer resorts and many tournament class golf courses. There is a Casino with regular scheduled Los Vegas quality shows and concerts. This metro area has 156k people living in the city and 456k in the county. There is a international airport with direct jet service to most major cities in the US. High quality education available. There is a large selection of inexpensively priced private schools. Some schools, both private and public, are nationally renowned for their academics and sports programs. There many colleges and universities in the region with Division One ranked football, basketball & lacrosse programs. As the number of encounters increase performance based compensation may increase to a maximum of 30% of the Adjusted Target Compensation. Dissemination of the information is reviewed quarterly by a Third Party to maintain the integrity of the process. * JOB TYPE is Permanent/Retained Employee, Full-time. * WORK SCHEDULE is Monday through Friday, approximately 8am to 4:00 pm, which totals approximately a 36 hour work week. * LICENSE and EDUCATION required is a application for or possession of a New York State license. NEW GRADUATES are encouraged to apply. * JOB DESCRIPTION: Provide comprehensive medical services for members of family, regardless of age or sex, on continuing basis by performing the following duties. Essential duties and responsibilities include the following. Other duties may be assigned: * Review patient chart/record for allergies, medical problems, medications, and immunization status. * Elicit and record information about patient's medical history. * NOT REQUIRED to perform labs. All required labs may be sent out. * Examine patients for symptoms or physical information. * Order or execute various tests, analysis, and diagnostic images to provide information on patient's condition. * Analyze reports and findings of tests and examination, and diagnoses condition of patient. * Administer or prescribe treatments. * Determine and prescribe medication, dosage, and schedule given the patient's condition and allergies. * Discuss any possible side affects to medication or immunization with patient. * Prescribe and administer vaccinations to immunize patient from communicable diseases. * Promote health by advising patients about diet, hygiene, and methods for prevention of disease. * NO prenatal care to pregnant women, delivers babies, and provides postnatal care to mothers and infants. * NO REQUIRED surgical procedures commensurate with surgical competency. * Refer patient to medical specialist for consultant services when necessary for patient's well-being. * Document the patient's visit including medical history, physical exam, diagnoses, and plan of action. * Follow up with patient regarding progress in high risk or emergency cases. * Conduct physical examinations to provide information needed for admission to school, consideration for jobs, or eligibility for insurance coverage. * BENEFITS: 1.) HEALTH INSURANCE is a Blue PPO Plan provided by Excellus Blue Cross & Blue Shield. The co-pay. for office visit $20. There is no deductible. Employee contribution is $32.50 for Single coverage and $130.00 for Family coverage.
