As a Case Manager you will:
Develop programs for specialty subacute / transitional care unit programs in designated hospitals or facilities through education to the community including healthcare providers and managed care organizations, Conducts on-site physical evaluations or collects clinical data of potential subacute candidates for admission to the specialty program, Conduct Off site evaluations for potential transitional care candidates on a case by case basis, provide accurate family information to social worker

Experience: Must be Licensed or Certified.
