Set your own hours! Jacksonville, Florida physician need for Medicare Home Assessments - no Rx / no patient treatment. Any specialty: family medicine, internal medicine, general surgery, general practitioner, etc. Provider must do online training to learn how to complete the assessment form. Average assessment takes 45 minutes; on average 6 to 7 assessments can be completed in one day. Assessments can be scheduled 7 days per week with start times as early as 9am and as late as 7:30pm. Provider's schedule can chose to schedule two or more assessments per day. Provider will receive a schedule according to availability of appointments within the same zip code in Jacksonville, FL. Provider must drive their own vehicle from patient home to patient home to conduct an in-person assessment. Providers are paid $60 per evaluation by Mint Physician Staffing within 14 days of verification of completed assessments by client. Mileage will not be reimbursed. Completed assessments must be sent via secure fax, email, or delivered to client's offices in Jacksonville, FL.
