Educational Qualifications

· Graduate of an accredited School of Pharmacy with Pharm.D. or B.S.

· Current license in respective state of practice

· Minimum 1 year experience in pharmacy with own license, hospital, parenteral home care service, or closed-door pharmacy experience preferred
