Responsibilities and Requirements: PT Housing Services Case Manager provides case management, advocacy and support services to clients seeking or participating in AIDSCare Phoenix Suppportive Housing Program and other AIDSCare housing programs on the West Side of Chicago. Applicants to this second shift (2p-10p) position will need to have a current drivers license. Primary responsibilities include providing intensive case management for AIDSCare Phoenix Clients, coordination of social service intake and assessment, and social service support of the Phoenix resident's ability to live independently.
