Schedules pre-admission interview with prospective resident and/or appropriate family members or sponsor after inquiry is completed. Verifies resident's 3 day hospital stay, resident's insurance coverage of planned care and documents covered days available. Assesses appropriateness of resident admission. Presents completed inquiry form to Administrator/Director of Nursing for admission approval. Coordinates admission of resident with Medical Social Services/Outcomes Management and transferring facility/unit. Reviews Medicare Skilled Nursing Facility Care coverage requirements with the residents insured by Medicare. In forms the resident or responsible party about co-insurance. Ensures that required admission papers are completed in a timely manner and required signatures are obtained. Provides financial counseling as appropriate. Refers residents and/or family to appropriate agencies, community resources etc. Explains all criteria or regulations which per unit policy to the resident and/or family. i.e.; Personal items, visiting hours, non-covered charges, resident rights. Completes and records the Social Service interview and assessment within 7 days of admission and develops a plan for meeting these needs. Completes designated section of MDS Face Sheet and Assessment within specified timetable. Documents in resident's chart and in Social Service files so as to reflect how the psychosocial developmental needs of the resident are being addressed/met consistent with the plan of care. Interviews residents on a regular basis to determine psychosocial needs, changes, progress etc. Coordinates care plan conferences. Assess the resident's needs presents a social service care plan from the MDS to develop the resident's goals for the written care plan. Assists residents and families with their adjustment to the facility, promotion of continuity of family relationships and encourages residents in reaching their rehabilitation potential. Assist physicians and staff in gaining a better understanding and accepting the resident's illness and/or disability. Assures that a positive and psychosocial therapeutic environment is maintained through staff training and input on policies and procedures. Ensures that initial discharge plans for each resident are initiated upon admission and documented. Includes indications of the duration of each needed service and expected length of stay in the facility on the discharge plan. Coordinates weekly discharge meetings. Updates and reviews the resident's discharge plan with input form resident's physician, nursing staff, each discipline involved with the resident, family or responsible persons and representatives from the community resources. Reviews discharge plan and updates whenever resident's care plan is revised. Reviews resident's eligibility for continued stay in timely manner. Notifies resident and/or responsible family when Medicare or other insurance coverage is about to end. Works with staff in developing and coordinating a home care program designed for continuation of care and progress. .
