Responsibilities: Responsible for the evaluation, planning, and administering of physical therpay treatment as prescribed by a licensed physician. The focus of Acute Care Physical Therapy with daily treatment, is to help the patient prepare to leave the hospital setting as soon as possible and hlep determine if patient would benefit from Transitional Care (SNF), Home Health or Outpatient Therapy when they leave. The focus of Transitional Care (SNF) Physical Therapy, with usually 5x week treatment is: 1) Helping the patient prepare to leave the TCU setting for home or another residence with the level of caregiver assistance needed; or 2) Helping patients who stay on in TCU setting, to optimize recovery and/or compensatory strategies for their functional mobility daily living activities. This may include direction & oversight of Restorative Aide. .
