Th efoolwoing opportunity is available immediately!CASE MANAGER II Position Purpose: Support, plan, provide medical resources, and coordinate care for State of Wisconsin Medicaid recipients. Members identified with high risk factors are provided support through case management to achieve optimum outcomes. Develop, assess and adjust as necessary the care plan to promote desired outcomes. Coordinate services between Primary Care Physician (PCP) specialist, and other medical and non-medical providers as necessary to meet the complete medical socio economic needs of clients. Provide patient education. Identify related risk management quality concerns. Evaluate options and services to promote quality, cost effective outcomes. Knowledge/Experience: RN with pediatric and medical surgical experience. Prior managed care or case management experience preferred. Two or more years of clinical experience. Licenses/Certifications: WI RN license required. Associate or Bachelors Degree. Current drivers license required.
