Responsibilities: The Specialty Leader CM will assess, plan, implement, coordinate, monitor and evaluate options and services to meet a patients need within an assigned case load, using available resources to assist in achieving an effective discharge plan while promoting quality health care with cost effective outcomes. The Case Manager also coordinates and completes utilization review activities and assures compliance with federal, state and third party payers. He/She participates in performance improvement initiatives related to the management of patient care as appropriate and performs other departmental duties as assigned. Must have previous case management experience in an acute care hospital setting..
