Responsibilities : This position is responsible for serving persons with developmental disabilities by supporting them and their families in leading the most healthy, self-sufficient and productive lives within their capabilities; performs a variety of tasks in case management for clients with behavioral health issues that are in a psychiatric setting, or clients entering/leaving psychiatric settings on a recurring basis which requires intense coordination of care, so that the return to the community is successful; gathers all necessary information and documentation for determining eligibility and/or re-determining eligibility for agency services; acts as the Lead worker in the Unit and carries more complex and high profile cases; serves as back up Supervisor; and performs other job related duties. In state travel required. Additional Job requirements for employment are: a valid Arizona driver license, valid motor vehicle insurance, current Arizona vehicle registration. Employees may be required to use their own transportation; however, travel expenses will be reimbursed.

Requirements :Applicable governing directives pertaining to social services; placement, custody and treatment of children and adults, community resources, court practices involving custody and placement, common human growth, personality and behavior; developmental and behavioral problems of children, their causes, symptom treatment, effects of foster care, adults of advanced years and cultural, environmental and community influence on individuals. SKILL in: oral and written communication; interpersonal relations with clients, families, court representative to community agencies; to: perform effective case management, interview, elicit information, counsel and interact with clients and families; manage caseload; organize; diagnose case problem needs and evaluate suitability of resources; provide work leadership.
