We invite you to explore career opportunities with and help us fundamentally change the way healthcare is delivered!:The Nurse Case Manager, (CM) works face to face with the member, the member family and providers involved in the member care to coordinate the services necessary to help reduce preventable hospitalizations. The CM is responsible for developing a plan of care for the member in conjunction with clinicians, and the members PCP and specialist.Provides face-to-face and telephonic Case Management to assigned membersDevelops an individualized plan of care and implements the planCommunicates with nursing staff, physicians, nurse practitioners and other health workers involved in the care of a memberCoordinates with member, family, and caregivers to help resolve barriers to careEvaluates member/family strengths, health behaviors, and resourcesIntervenes with at-risk members to avoid unnecessary hospitalizationsSuccessfully collaborates with member and family to assure comprehension of available resourcesConsiders member/family physical, educational, and financial resourcesProvides on-call services, and travels between facilities as requiredOther duties as required.

Requirements :Registered Nurse with 3-5 years related experience and/or training in Case Management, or Licensed Practical Nurse with 5-7 years related experience and/or training in Case ManagementSocial work training, licensure is highly desirableMedicare experienceExperience with geriatric populations and community resource organizationsExperience handling confidential health care informationExperience with care management software applicationsExperience with / exposure to the case management of transplant candidates and recipientsExcellent interpersonal and communications skillsAbility to work within Microsoft Office applications.
