Responsibilities: Works in collaboration with physicians, nursing and ancillary services to coordinate and assist with implementation and evaluation of the patientÆs plan of care. Responsible for facilitating the process of utilizing available resources to achieve the best possible patient outcomes. Case managers have a specific caseload or mix patients. A BachelorÆs Degree in social services field or other comparable clinical degrees. Current registration as a Registered Professional Nurse in the State of Illinois. Minimum of three years clinical experience in the area of clinical specialty. Well developed communication and organizational skills. .
