Responsibilities : RN Case Managers Lee Memorial Health System- Lee County, Florida (s available in our 4 hospitals- We are expanding our services in our emergency departments) Emergency Department Case Managers perform utilization management functions to ensure medical necessity and the appropriate setting for patient care. Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual health needs throughout the continuum in various settings through communication and available resources to promote quality cost-effective outcomes. Provides emergent clinical interventions and crisis management and discharge planning through assessment and counseling. Expedites linking patient with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning. - This is a 6-12 hour shift 8 hour shift and every other weekend. Dec-Mar 12 hours.

Requirements : As or above in health related profession with plans to obtain Bachelors Degree within 5 years. Experience: 3 years clinical experience. Case Mgmt experience preferred.
