Responsibilities: Primary Care Medical Home (PCMH) Steering committee, based on internal priorities and payer requests. Uses patient registries, information technology, health information exchange and other means of communication and data gathering to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner; Performs ongoing communication with the patient/ family and care providers and payer to ensure that care is coordinated and patient needs are met. Aware of payer stipulations for care provision and works with the payer to ensure patient care needs are met. In conjunction with the PCP and Lead physician is responsible for monitoring and reporting outcome indicators for the Medical Home. Will work at both the OSFMG Knoxville Avenue Pediatric and OSFMG Washington Internal Medicine/Pediatric pratices. RN required/ BSN preferred. Self management certification/ training must be completed in 1st 6 months of hire. Broad knowledge of medical and community resources. 3-5 years of physician office or case management experience. Positive, empathetic approach and ability to communicate effectively to patients/ families with a variety of health issues, age groups and socioeconomic backgrounds. Flexible adaptable teaching/ coaching methods based on patient's learning needs and barriers. .
