Responsibilities : The Perinatal Care Coordinaton Registered Nurse Case Manager will provide case management and education to members enrolled in the Perinatal Care Coordination Program. The RN Case Manager will be a key component in achieving the overall goal of enhancing clinical outcomes, reducing member/baby hospitalizations, and reducing healthcare costs to clients.Duties:Overall -Conduct home visitations for high risk perinatal memebers.-Manage the assigned member caseload according to policy and procedure.-Conduct telephonic coaching and follow-up services.-Perform timely and accurate re-assessments.-Assist with problem identification, goal setting, decision-making, problem-solving, and addressing priority needs.-Communicate effectively with OB provider for approval of member-centered plan of care.-Maintain current member records in the data management system.-Communicate effectively with all members of the Perinatal Care Coordination Team.-Accept related assignments as directed by the supervisor.-Travel through the western part of southern Indiana required.

Requirements :Must possess a Baccalaureate/Associate Degree of Nursing from an accredited college or university.Must possess or be able to obtain a valid Indiana RN license.At least one year of strong clinical experience.OB/L&D/Perinatal experience preferred.Excellent verbal and interpersonal communication skills.Possess knowledge of problem identification and problem solving processes.Must have reliable transportation and auto insurance.
