Responsibilities : Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of functions of case management, utilization review and management and discharge planning.Location:Kindred Hospital - Indianapolis South 607 Greenwood Springs DriveGreenwood, IN 46143 Coordinates management of care for a specified patient population; follows patients throughout the continuum of care and ensures optimum utilization of resources, service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of functions of case management, utilization review and management and discharge planning.

Requirements :Healthcare professional licensure preferred as Registered Nurse, LPN, Respiratory Therapist, Physical Therapist, Occupational Therapist or Social Worker required, Registered Nurse (RN) or Licensed Practical Nurse (LPN) strongly preferred.2. of three years clinical work experience in acute care or long-term acute care settings required.3. Previous work experience with Case Management, Utilization Review, Discharge Planning, and/or Quality Management strongly preferred.
