Responsibilities : The position of Case Manager contributes to the mission of the Medical Center and the goals of the department by being accountable for coordinating, negotiating, procuring and managing the care of identified patients to facilitate achievement of quality and cost patient outcomes. Participates in quality improvement and evaluation processes related to the management of patient care.
