Requirements: 1-2 years acute care experience is required - Basic computer skills required - Customer service skills are required Under the direction of the Clinical Supervisor and within established agency policy and procedure, the WOCN Coordinator will: Provides superior customer service to internal and external clients, customers and patients as referenced in the Service Excellence Standards for AEC Reviews and evaluates all wound patients Teaches/mentors clinical staff regarding wound care, providing consultations when needed Evaluates need for reintroduction of skin care and other specialty teams; assists with implementation of recommendations Empowers staff to care for ostomy by providing comprehensive education Assumes accountability for own professional practice in achieving patient outcomes Accurately, promptly and thoroughly documents patient care observations, interventions and evaluations Works collaboratively with scheduling and PI departments Researches and recommends wound care/ostomy products that are of excellent quality and cost-effective to achieve optimum patient outcomes Caritas Home Care's mission finds expression and is routed in the core values of Compassion, Humanity, Excellence, and Commitment.
