Job Summary:
The NP performs ongoing clinical assessments, establishes and implements plans of care for acute and chronic conditions of patients.
The NP manages preventive health aspects of care for the patient and evaluates that plan for effectiveness.
The NP assesses the patient for learning needs and strategizes to meet those needs for the patients and their families.
The NP independently plans, develops and implements a plan of care and evaluates periodically.
Adjusts plan accordingly.
Strives for patient centered care in all aspects of the clinical environment.
Responsible to independently diagnose and treat complex health conditions of the body and mind.
The NP regularly collaborates with physician and periodically participates in chart and coding review.

Summary of Responsibilities:
APNs may provide direct patient care to a selected patient population through the following activities:
Independently conducts comprehensive and episodic health assessment by taking histories, performing physical examinations, and generating SOAP note.
Requests and interprets appropriate lab and diagnostic studies and formulates a plan of care including pharmacological and non- pharmacological treatment.
Performs clinically appropriate diagnostic and therapeutic procedures within the scope of practice and expertise as defined by position description.
Consults with specialists as needed to accomplish collaborative care.
The NP is expected to share knowledge to empower patients, families and colleagues.
Exhibits organizational, communication, leadership skills, initiative, accountability, responsibility, appropriate judgment and decision making.
Maintains educational credentials appropriate to the work environment.
Conducts annual well visits in accordance with the CMS guidelines and make appropriate referrals.
Clearly educates and guides patients on DX prevention and healthy life style habits.
Seeks to empower patients to participate in decisions regarding their care.
Understanding the changes in health promotion throughout the aging process.
Prescribe medication as per state law and Medical Facility policy.
Assesses and refer accordingly for social and behavioral health needs.
Utilizes national guidelines and Medical Facility established protocols to care for patients.
Keep current regarding immunizations, care guideline and preventive screenings.
Reviews patient results and independently develops subsequent plan of care.
Ensures results are to be communicated timely to the patient according to policy, and documented appropriately.
Responds to patient calls and messages according to policy.
Demonstrates excellent writing and verbal communication.
Utilizes collaborative communication and demonstrates a team approach to patient care.
Is a team leader in clinical setting empowering and mentoring the staff to support the clinical environment.
Documents timely and comprehensively to ensure the patient record reflects and actively participates to improve patient care for the practice and the organization.
Reviews patient discharge records, facilitating transitional care management and responds appropriately and timely with the needed follow up.
Conducts advanced care planning in a sensitive and caring manner.
Participates in on-site performance improvement/patient satisfaction initiatives.
Participates in population health strategy, appropriately risk stratifying with focus on triple Aim.
Other duties and responsibilities as assigned.

Minimum Certifications, Registry and/or Licenses Required:
CDS, DEA, NJ RN License, APN Certification and Licensure

Minimum Degree Required and Experience Required:
Masterís Degree, and national board certification as a nurse practitioner.
Strong listening and verbal skills.
Consistent communication with physician(s) with any patient issues/concerns.
Basic computer skills.
Familiarity with HIPAA and patient confidentiality regulations.

Working Relationships:
Physicians, Patients/Families/Visitors, Administrators, Outside Vendors and Office Staff.
