Job description
Position

Provide quality patient care through the development, implementation & evaluation of individual care plans. Chief liaison staff member between HFM Pavilion, HFM Sub-Acute & Short Term Rehabilitation Unit, interdisciplinary team, residents and families. Follows established professional standards of care and HFM policy, procedures and clinical pathways. Oversee the provision of quality clinical care, wound management and administration of standards, policies and procedures. Main areas of responsibility include the formulation and revision of policies and procedures; facilitate mandatory and non-mandatory in-services, and ongoing professional development of all staff.

Duties and Responsibilities:

Assistant Director of Nursing

1. Assist Director of Nursing in maintaining a highly successful nursing team.

2. Develop and periodically update statements of philosophy and objectives that define the type of nursing care the facility proposes to provide. The objectives identify the importance of integrating both the plan of medical care and the plan of resident care.

3. Develop and maintain nursing service policies and procedures to implement the program of care.

4. Ensure that all of the nursing needs of residents are met by assigning a sufficient number of qualified supervisory and supportive nursing personnel for each tour of duty.

5. Participate in coordination of resident care services through use of the interdisciplinary team’s assessment and care plans.

6. Participate in planning and budgeting for nursing service.

7. Review the nursing requirements of each resident admitted to the facility and assist the attending physician in planning for the residents’ care.

8. Assume responsibility for maintaining their own professional competence through participation in programs of continuing education e.g. nursing seminars and short-term training courses.

9. Ensure that the philosophy and objectives are understood by all nursing personnel.

10. Assign duties and monitor and evaluate work performance of subordinate nurses and other nursing staff as indicated.

11. Assist in developing a Plan of Care that corresponds with the physician’s orders through assessment of patient's needs, condition, environment, and consultation with other interdisciplinary team members from admission to discharge.

12. Participate in weekly Quality Measure Meeting.

13. Participate in SBAR Meeting.

14. Participate in Utilization Management Meeting.

15. Oversight of Restorative Nursing programs on Nursing units.

16. Oversight of Electronic Health Record and Informatics.

17. Responsible for Accident/Incident Reports chemical and physical restraints on Nursing units.

18. Direct, in supervisory capacity, the HFM interdisciplinary team in meeting the patient care plan goals.

19. Maintain a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops and professional organizations.

20. Respond to and facilitate emergency procedures.

21. Respond to issues, problems as referred by Unit Supervisor/Charge Nurse and staff.

22. Perform investigations according to HFSL policies and procedures, knowledgeable with Department of Health (DOH) reporting procedures.

23. Formulate and revise policies and procedures; educate staff on policies and procedures, including tracking and routing new and revised policies.

24. “On Call” rotation for Nursing Administration as determined by the Director of Nursing and/or the Director of Clinical Operations.

25. Provide coverage for House Supervisor, as needed.

26. Understand and comply with all federal, state and local regulations and requirements.

27. Perform other duties as assigned.

Clinical Education

28. Coordinate and monitor clinical competency programs for licensed staff in accordance with regulatory requirements.

29. Cooperate with the administration in planning a staff development program that will upgrade the competence of the personnel. Specific attention is given to improve supervisory skills of the charge nurses and the multi disciplinary approach to resident care.

30. Formulate and revise policies and procedures; educate staff on policies and procedures, including tracking and routing new and revised policies.

31. Plan and provide Director of Clinical Operations/Reimbursement and DON with an annual schedule of education programs.

32. Coordinate and monitor staff education including compliance with mandatory and non-mandatory in-services; oversee the entire education plan and maintains documentation

The ideal candidate for the ADON position will have previous experience in a nursing leadership role, a strong ability to work and collaborate with a team, and an in-depth knowledge of managing and training nursing staff through implementation of quality improvement programs.

JOB REQUIREMENTS

Education and Experience:

BSN required; MSN preferred.
3+ years of nursing experience is required.
2+ years of ADON nursing leadership experience required
Knowledge of state and federal regulations pertaining to LTC facility required.
Equivalent combination of education and experience which provides the required knowledge, skills and abilities as cited above will be considered.
Other Requirements:

1. Must read, write and speak the English language.

2. Possess the ability to make independent decisions when circumstances warrant such action.

Additional Information

What’s the average patient census? We are a 208-bed skilled nursing (148 beds)/short-term rehab (60 beds) facility. Our census is currently 136. We are COVID free!
What’s the employee turnover rate or employee vacancy rate? Our turnover rate varies typically 1 -7% range. Our turnover rate for July was 1.66. We have many long term employees who have been with HFSL 10 – 30 years!
Are you using travel or agency nurses? Yes, currently have two contracted local agencies with whom we have daily/weekly communication. The goal, of course, is to eliminate the use of agency staff.
Is there a ADON and Unit Manager (s)? Yes, we have all of the above! We also have an (RN) House Supervisor on each shift.
Who does the Infection Control ? We have a full-time (RN) Infection Preventionist on staff.
The ADON must have prior ADON experience and strong leadership skills. The CEO and the President of their Board of Directors are both RNs, so candidates for this position must be polished
Required Licenses/Certifications

Licensed Registered Nurse within the Commonwealth of Pennsylvania.
