JOB SUMMARY:Responsible for training and assisting individuals served in various aspects of activities of daily living, while ensuring compliance with regulatory requirements and company policies and procedures. This position is also responsible for the health, safety and emotional support of the individuals being served. This is a group home setting with 4-8 people served in the home. The staff will be responsible for the ADL's. ESSENTIAL JOB FUNCTIONS:Provide training in accordance with the written program plan, covering all aspects of the individual s life as assigned and in compliance with company policy and procedureMonitor the comfort and safety of individuals served, while ensuring that the medical, nutritional and personal care needs are being metSupport people served to identify and achieve the personal outcomes important to themAttend and participate in the team program plan meetings as assigned and regularly communicate each person s progress with team members, supervisors, parents and/or guardians, as appropriateInteract with individuals in an emotionally supportive and therapeutic manner, and practice effective techniques for reinforcing appropriate individual behaviorMaintain medical, social and programmatic files by recording each person s progress and documenting other required informationProvide transportation and assist individuals with integration into the community by accompanying them to social and spiritual activitiesAssist each person served with general housekeeping, meal preparation and laundry dutiesComplete assigned cleaning tasks on a routine basis in order to maintain a neat and clean environmentAssist individuals in handling their money and financesProvide appropriate leisure time activitiesKNOWLEDGE, SKILLS & ABILITIES:Knowledge of Company policies and proceduresKnowledge of and ability to demonstrate Mosaic s 7 Essential Practices of LeadershipBasic knowledge of nutrition, housekeeping, meal preparation, personal hygiene and first aidSkill in working with individuals with developmental disabilitiesAbility to communicate effectively in a courteous and professional mannerAbility to teach individuals with developmental disabilitiesAbility to read documents and written instructionsAbility to write and maintain accurate recordsAbility to maintain confidentiality.Ability to stoop, kneel and lift to complete assigned tasksAbility to use the telephoneAbility to maintain a patient and positive attitudeAbility to drivePHYSICAL REQUIREMENTS:Work requires frequent physical activity including extended periods of standing, walking, and bending with occasional periods of sitting, kneeling, climbing, stooping, crouching, squatting and balancing. Work also requires constant reaching between knee and shoulder level as well as frequent reaching below knee level and overhead. Work requires occasional independent lifting up to 25 pounds, frequent push/pull up to 40 pounds of force and the ability to safely transfer 50 pounds.EDUCATION & EXPERIENCE:High school diploma or equivalent preferred. Minimum of three months of experience in a related field. Must be at least 18 years of age. NE Medication Aide Certification.
