Surgery-AdministrationHours : 8a-5pThis person will work exclusively with our Bariatric surgery team. Assists in maintaining flow of patient care and serves as clinical liaison with all interdisciplinary providers, referring physicians and patient care coordinators/navigators. Contacts patients to deliver test results and follow up instructions. Updates and audit patients health records for accuracy and thoroughness. Ensures all needed documentation is acquired for care planning and insurance authorization. Participates in quality improvement activities for assigned programs. Contacts patients after hospitalizations, urgent care, emergency room visits, and no-shows and schedules follow up appointments as indicated. Coordinates high risk patient appointments. Serves as a clinical resource for medical assistants and LPNs. Administers vaccinations and medications. Participates in the development of and evaluates the outcomes of educational programs for patients/families. Role models clinician behaviors that demonstrate specific professional values and commitment to patient population. Provides consultation with internal and external physicians, other health care providers, and health plan and regulatory agencies to enhance and monitor the quality of care and the level of care provided for assigned patients. Helps educate colleagues and work within the facility to encourage documenting and using preferred pronouns, gender identity and gender expression. Maintains up-to-date knowledge of current activities in transgender care and recommend/implement enhanced clinical practice standards for the department and the region. Provides medication reconciliation and adherence counseling, while assessing patient knowledge and barriers to adherence and self-care. Collaborates with multi-disciplinary team to address both physical and psychological aspects of health to provide care for the whole patient. Accesses appropriate resources inside and outside the organization to meet the needs of the patients PA Registered Nurse License (Required)
