Forensic Treatment Agency seeking Navigator/Drivers for the Rapid Diversion program. The Navigator/Driver will be responsible for providing support to clients being diverted from court and help transport them to various housing, treatment providers, or other appointments. Ideal candidate will be interested in working with the justice involved and/or mental health population. Personal car is required for the position and mileage reimbursement is provided.


Essential Functions

Ÿ Individual case management for clients as needed by exploring community resources, providing linkage, and referrals to ensure continuous system of care

Ÿ Coordinate services with various public and private community health, mental health, and social service agencies to ensure appropriate services for clients

Ÿ Transport client(s) to connect with necessary community resources

Ÿ Participate, coordinate and implement cooperative arrangements and/or team conferences with various inter-disciplinary team members in the development and implementation of client’s treatment plan and aftercare plan.

Ÿ Coordinate and implement group behavioral treatment activities as needed for the Project 180 programs under direction of Clinical Supervisor and/or Program Supervisor/Manager

Ÿ Maintain and update client’s files and documentation in accordance with DMH or DHS contract guidelines

Ÿ Pick up/Drop off of Project 180 clients

Ÿ Supervision of clients while in the community or at appointments

Ÿ Communication/coordination with Project 180 staff regarding client transportation needs

Ÿ Crisis intervention/management as needed while Project 180 clients are in van or community under supervision of the driver

Ÿ Communication with treatment staff regarding observations of clients to assist with client care

Ÿ Care/upkeep of the Project 180 vehicle including regular maintenance, cleaning, parking, etc.

Ÿ Professional development and education as needed
