AlediumHR, one of North America's leading telehealth and healthcare recruiting firms, is actively recruiting a PACE Primary Care Physician to provide comprehensive medical care to seniors in their homes and communities. You will work closely with a team of healthcare professionals to develop individualized care plans that meet the unique needs of each participant.

Core Responsibilities:

• Perform in-person comprehensive assessments and develop care plans for participants.
• Collaborate with community-based physicians and the integrated discharge team (IDT) to coordinate care.
• Oversee all care of participants in nursing homes and hospitals.
• Provide direction and support to Nurse Practitioners and other members of the IDT.
• Take part in rotating night and weekend calls.

Core Requirements:

• Doctorate in Medicine.
• Board certification in internal medicine or family practice.
• Active California physician license without restrictions.
• State CDS certification and DEA registration.
• Valid state driver's license with a clean DMV record.
• A copy of recent vehicle insurance is required.
