Licensed Social Worker (LICSW or LCSW)

About the Role:

• Focus exclusively on the geriatric population

• Conduct psychosocial assessments and create individualized care plans

• Participate in daily interdisciplinary team meetings

• Coordinate facility admissions and discharges as needed

• Educate and support families with community resources

Qualifications:

• Minimum of 1 year of experience in Social Work providing traditional clinical or case management services with a geriatric population

• Active LICSW or LCSW license required

• Strong communication and teamwork skills

• Compassionate approach to supporting older adults and their families
