Overview

Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in patient care across diverse settings. As an LVN, you will provide compassionate, skilled nursing services, ensuring the highest standards of safety, comfort, and health for our patients. Duties

Deliver direct patient care by performing physical examinations, vital signs assessment, and comprehensive patient evaluations.
Administer medications, injections, and other treatments following strict protocols and medical documentation standards.
Assist with complex procedures such as catheterization, tube feeding, and sterile dressing changes while maintaining aseptic techniques.
Manage electronic health records (EHR/EHR systems) accurately to ensure seamless documentation of patient care activities and medical records.
Coordinate patient care plans through case management and discharge planning, collaborating with multidisciplinary teams to optimize outcomes.
Support specialized care initiatives including dementia, Alzheimers disease, memory care, wound care, medication management, and working with individuals with developmental disabilities or geriatric patients.
Requirements

Valid licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of employment.
Demonstrated proficiency with EMR/EHR systems such as DSL, Epic, Cerner, or eClinicalWorks.
Strong knowledge of medical terminology, anatomy, physiology
Experience with medication administration including injections; familiarity with sterile processing and infection control protocols.
Ability to perform physical examinations and assessments on diverse populations including geriatric patients, bed bound, limit mobility patients.
Excellent communication skills for patient education on health coaching topics like pain management or nutrition; capable of working independently and within team environments.
