Job Description:
- Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care)
- Verify and enter physician orders
- Identify level of care needed
- Receive and give verbal orders accurately
- Review clinical documentation to determine Skilled Nursing placement
- Exceptional interpersonal and customer service skills
- Ability to communicate clearly with medical providers
- Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
- Manage after-hours and weekend referrals (rotating on call)
Skills & Qualifications:
- Current license as a licensed LPN/RN from the Utah State Board of Nursing
- 1-year Skilled nursing facility experience preferred
- Good verbal and written communication skills
- Basic computer knowledge
- Ability to effectively problem solve
- Excellent customer service skills
- Knowledge of Medicare/Medicaid Skilled Nursing placement criteria preferred
