PRIMARY POSITION PURPOSE:

The Family Support Coordinator will provide information about health home options or alternative community support resources. Family Support Coordinators are the second line of support for children and their families after the Independent Evaluator assesses the family for eligibility and develops a consumer driven Plan of Care.

EDUCATION:

• Preferred Education Bachelor’s Degree of Social Work or other mental health specialization degree from an accredited college or university


• Required Education Bachelor’s Degree from an accredited college or university

QUALIFICATIONS:

• Required Experience 1-3 years of experience working with children with behavioral health disabilities/mental health as a Case Manager


• Required Experience Working experience with pediatric psychiatric patients.


• Required Skills Computer literacy; knowledge of MS Excel; ability to learn new software programs quickly and intuitively; excellent organizational, interpersonal, written, and verbal communication skills; ability to perform comfortably in a fast paced, deadline-oriented work environment; ability to successfully execute many complex tasks simultaneously; and ability to work as a team member, as well as independently.
