Job Description

You will provide assistance in daily living and personal care services in patients residence or care facility in accordance with an established care plan.

 

Responsibilities:

•Provide routine individualized care in clients residence

•Assist client with transfers, mobility and activities of choice

•Assist client with personal care, hygiene, bathing & dressing

•Perform domestic household tasks and meal planning & assistance

•Provide companionship and basic emotional support

•Transport and/or accompany patient to doctor’s office, shopping or to hospital as needed

•Maintain patient care records, document provided services and monitor & report changes in health status

 

Qualifications:

•Proven working experience as Companion, Home Health Aide or CNA

•Caring and compassionate personality

•Experience in hospice, home health, or other related fields

•Positive and patient demeanor with an ability to build rapport with clients

•Excellent written and verbal communication skills

 

Summary of Details:

Multiple shift options available with 4-hours minimums, 8-hour, 12 hour, 24/7 live in

or nights and weekends only cases.
