We are looking for compassionate Home Health Aides / CNAs to become a part of our team! You will assist in daily living and personal care services in patients' residences in accordance with an established care plan. Cases throughout Monmouth and Ocean Counties.

Responsibilities:

•Provide routine individualized care in clients residence

•Assist client with transfers, mobility, and activities of choice

•Assist client with personal care, hygiene, bathing & dressing

•Perform domestic household tasks and meal planning & assistance

•Provide companionship and basic emotional support

•Transport and/or accompany the patient to the doctor’s office, shopping or to hospital as needed

•Maintain patient care records, document provided services and monitor & report changes in health status
