JOB SUMMARY:Promotes and maintains health of enrollees within the LIFE Geisinger Center and community through teaching, counseling, and appropriate preventive and rehabilitative nursing services.Responsible for assessments of health needs and contributing to the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic reevaluations of individual and family nursing needs. Demonstrates the knowledge and skills necessary to assess, plan care for and provide service to the frail elderly participants according to assigned responsibilities and LIFE Geisinger standards.Reports to the Nurse Specialist, LIFE Geisinger, Geisinger Community Health Services.SKILLS AND ABILITIES:Demonstrated excellent clinical, oral and written communication skills. Must be flexible, adaptable, and able to function in stressful situations.Demonstrated good interpersonal skills and ability to foster teamwork; Ability to triage work effectively required.Must be able to coordinate multiple projects simultaneously and bring them to a timely and accurate completionEDUCATION AND/OR EXPERIENCE:Must maintain a current RN licensure within Pennsylvania. Bachelor Degree preferred.A minimum of one-year experience required in working with an elderly population. Outpatient clinic office experience preferred.A minimum of two-years experience in clinical nursing is required.
