Position Overview The Care Management (CM) Associate will perform in home member assessments as part of the care management team. The CM assistant participates in assessing the needs of the members, and works with the rest of the team in coordinating care and implementing person centered service plans. This position is a field based position. The work is carried out in support of the mission and goals of Independence Care System. The salary range for this fulltime position is $40,000 - 45,000 per year
Essential Functions 1. Conducts daily care management home visits for assigned members. - Prepares for member visits by reviewing case materials, and bringing any pertinent materials needed for visit.
- Gathers all information needed to complete the home visit
- Reviews areas assessed and issues identified with the member during the visit
- Travel throughout the designated service area to meet with members as needed in alignment with regulatory standards and to ensure identified needs are met.
2. Member support in the coordination and provision of community based long term care services (CBLTC services) - Communicates and collaborates with Care Management and members regarding CBLTC services and any care needs
- Assesses family and community supports, home environment, social determinants of health and other safety concerns/needs
- Provide education based on assessment findings and hand off to care navigator for additional follow up and linkage if needed.
- Records and reports the outcome of the appointment to the Care Management team.
3. Collaboration - Participates with care navigators in developing and monitoring member care plan
- Communicates in a collaborative way with other ICS staff and members
- Facilitates case conferences with Health Home team and other internal and external stakeholders when necessary.
4. Reporting and Tracking - Documents all member related interactions and information thoroughly in the electronic medical record in a manner that reflects an understanding of member issues and needs.
- Participates in other activities related to regulatory compliance (i.e. MLTC projects/audits).
5. Other functions as specified in response to the needs of members and the organization, even if those needs do not coincide with the details of this job description Requirements Minimum Qualifications - Associates Degree required.
- Basic computer skills MS Word, Internet and Email.
- Able and willing to travel throughout Bronx, Manhattan, Brooklyn and Queens.
Additional Qualifications - Bilingual (English/Spanish or Russian) is a plus.
- Bachelors or Masters degree in human services, social work, or other related field a plus.
- Relevant field work experience preferred.
- Previous work experience in healthcare and/or non-profit a plus.
- Experience working directly with people with physical disabilities preferred.
