Title: Case Management Nurse

Description:



•    The nurses will mainly be based at our immunization stations in Annual medical vaccinating our membership and candidates.

•    They also need to be able to perform venipuncture competently

•    Reporting directly to the supervisor of nurses, the successful candidate will: -

•    Maintain a comprehensive case management system for the active members.

•    Document all the client encounters, including nursing's summaries, and evaluate and document the progress of individual therapeutic programs.

•    Confer with physicians to monitor treatment plans, report progress and analyze changing symptoms.

•    Coordinate initial assessments and ongoing reassessments of a client's functional status; and ensure a client medical need are addressed in accordance with established protocols.

•    Consult with physicians, as needed, to coordinate plans of treatment and advocate for a client, when necessary.

•    Participate in quality assurance and utilization review activities, including chart reviews and cheeks to ensure patient notes are accurate.

•    Maintain accurate and timely client information.

•    Review and ensure completeness of patient’s medical charts and data collection, and conduct follow up interviews with patient regarding any outstanding medical/mental health problems identified.

•    Duration:  It would be 2 -3 days/week and duration are for about 1 year
