Case Manager (Bilingual)

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 65,000 people in need through 100 programs across Long Island and NYC, seeks a Bilingual Case Manager for the Safe Harbour Providing Assistance Towards Healing (PATH) Program

The Bilingual Case Manager position is a non-exempt position paying $25-30 per hour. The hours of operation are Monday-Friday 9AM- 5 PM. The office is located in Riverhead, NY. The role reports to Supervisor.

Primary Purpose of Job:

The Case Manager is responsible to provide short and long term, trauma informed, case management and direct services for minor victims of human trafficking and youth at-risk of human trafficking that are referred to Safe Harbour. The Case Manager will be a part of the Suffolk County Child Advocacy Centers Multi-Disciplinary Team and will coordinate relationships between other MDT members and additional human services agencies as needed.

Principal Duties & Responsibilities:

  1. Complete risk-assessments for minor victims of human trafficking and youth at-risk of human trafficking that are referred to Safe Harbour
  2. Provide trauma informed, victim centered case management services and outreach to minor victims of human trafficking and youth at-risk of human trafficking to youth in Suffolk County, NY.
  3. Conduct assessments for client needs and develop an individualized service plan to support clients with safety planning, goal setting and psychoeducation.
  4. Document in the CAC NYSCA database the services provided to the client to ensure accurate case records are maintained.
  5. Collaborate with community partner agencies to deliver staff training regarding the identification of unaccompanied minors at risk of trafficking and connect then to services.
  6. Transport and escort clients to court proceeding, medical appointments and other appointments as needed.
  7. Assist with applying for public benefits, employment, medical care, return of personal property and short- and long-term coordination of such services.
  8. Make appropriate referrals for medical exams, mental health services, residential assistance,
  9. Maintain accurate database of professional referral resources that assist stated population.
  10. Prepare monthly and quarterly statistical and annual reports as required by funding source.
  11. Attend workshops and trainings as need for project implementation.
  12. Assist in preparing reports for the grant as needed.
  13. Participate in team meetings, case conferences, agency presentations, trainings and related events.

Knowledge, Skills, & Abilities Required:

  1. Masters Degree in Mental Health Counseling, Social Work, Forensic Psychology or Behavioral Health field required.
  2. Bilingual (Spanish Speaking) required.
  3. Minimum of two years of relevant experience.
  4. Must be proficient in computer applications or other automated systems such as Google Drive, spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  5. Excellent communication, organizational and interpersonal skills.
  6. Must have access to automobile and possess valid NYS driver's license.
