The Case Manager supports medical teams, improves clinical outcomes, and enhances patient satisfaction while managing costs. This role coordinates patient care, resource utilization, and discharge planning. The Case Manager identifies areas for improvement and updates relevant disease registries.

Minimum Qualifications

  • Education: BSN or MSW
  • Licenses: Valid RN or LMSW license in Texas; Certification in Case Management within 2 years of being hired
  • Experience: Minimum of 3 years in a clinical setting (i.e. ambulatory surgery center, infusion/dialysis clinic, Federally Qualified Health Clinic (FQHC), skilled nursing facility, or wound clinic), with preference for acute hospital-based experience (3 years)
  • Skills: Strong in communication, leadership, analytics, and data management

Principal Accountabilities

  • Coordinate patient care and facilitate discharge
  • Work with medical teams and solve system issues that impede patient care
  • Complete Utilization Management and Quality Screening
  • Monitor and improve length of stay (LOS) and resource use
  • Handle documentation and comply with payor requirements
  • Lead in performance improvement activities
  • Act as a mentor to new hires

The role demands strong organizational skills, the ability to work independently, and excellent oral and written communication skills.
