Requirements
The Case Manager (Registered Nurse) is a liaison between patients healthcare insurance, physician and Plaza Healthcare.
Responsibilities include: assuring patient care needs are met, ensuring medications are authorized, updating Insurance Case Managers with patient progress to ensure continued Insurance authorization of care. Also ensuring appropriate equipment is acquired, authorized and provided.
Responsible for all aspects of discharge planning i.e. Home Health, Community Resources and ordering DME as may be required by each patient. The case manager also assists family and facility in the ALTCS process.
This position reports to the Director of Case Management.
- Current Arizona RN License is required
- Case Management experience in a skilled nursing facility (1-2 years)
