Job purpose

The Case Manager will facilitate client wellness and autonomy through advocacy, assessment, communication, education, resource management, and service facilitation. This position will support the Director of Client Services in program development, implementation, and evaluation to further the mission and goals of the organization.

Duties and responsibilities

  • Conduct comprehensive assessments of clients to determine service eligibility, and needs.
  • Complete pantry check-ins, new client intake, update appointments and maintain accurate records.
  • Utilize a client-centered approach to provide ongoing case management to clients, including referrals to NKC educational programs and/or other community resources.
  • Attend all training necessary for continuity of client services: MDHHS Navigator training, MEAP, Casebook, and others as required.
  • Facilitate rental assistance, utility assistance, and all other services provided by NKC.
  • Manage client services empowerment programming through client recruitment and retention.
  • Coordinate formal and informal evaluations of client services and programming to ensure ongoing improvement.
  • Maintain accurate and up-to-date client records, maintain client database, and prepare monthly reports on client services as requested.
  • Ensure all client services policies and procedures adhere to the NASW Code of Ethics.
  • Serve on committees relating to NKC mission and promote NKC in the community.
  • Maintain regular contact with community service providers and stay abreast of new community resources.
  • Attend meetings and trainings as required.
  • Other duties as assigned by the Director of Client Services.

Requirements


  • Bachelors degree required in Social Work, Sociology, Psychology, or similar human services field required.
  • Fluency in Spanish is strongly preferred.
  • Minimum of one year of case management or comparable experience.
  • Ability to form and establish positive partnerships in the community.
  • Self-starter with a high degree of initiative.
  • Must work well in a team environment.
  • Have a good understanding of issues related to individuals and families that face socioeconomic challenges.
  • Must have an ongoing commitment to cultural competency.
  • Demonstrate strong organizational skills.
  • Must be creative and forward-thinking.
  • Excellent administrative skills; attention to detail and deadlines.
